Dear Special Education Department,
Specifically, we are about our students, because they struggle across all settings with academic performance, sensory integration, social skills, motor coordination, organizational skills, fine motor skills, working memory, and expressive communication. We are also concerned that they are not making progress commensurate with their potential. We want the following assessments done to support his current struggles:
Educational Assessment
Psychological Assessment
Social Emotional Assessment
In School ~ Functional Behavioral Assessment
Speech and Language Assessment
Occupational Therapy Assessment
Assistive Technology Assessment
Sensory Motor Assessment
Executive Function Assessment
If possible, I would appreciate the opportunity to discuss the assessment process further and to collaborate with the IEP team to ensure that we are taking into consideration all relevant aspects of their abilities and aspirations.
Additionally, I would like to be informed about the timeline and any required documentation or information needed to facilitate this process.
We understand that we must give written permission to be evaluated. We are happy to provide any information you need.
We would like a copy of all reports before our meeting so we may be a prepared team member at this meeting.
